2006 CERTIFICATION PROCEDURE - FOOTBALL

 

 

Sat August 19

 

Paperwork for all players with completed paperwork along with a draft of the “Official Football Roster” to be turned in to the Executive Director.  Only players whose paperwork is complete are to be on the roster.  The location and time will be announced.  If paperwork for individual participants is not complete, do not turn in. Players whose paperwork is not complete may not practice until after August 27th weigh-ins.

 

Teams should have their own clearly marked HANGING FILE FOLDER for each team.  The folder is to have the name of the association, division of play, and last name of the head coach.

 

 

Sat August 19

TEAM ELIGIBILITY:

 

·          Draft of the “Official Football Roster” provided by Pop Warner.  This roster is the only acceptable roster and the format is not to be altered.   Only players whose paperwork is complete are to be listed on the roster. 

·          Older/lighter players are to be listed first in alphabetical order followed by the remainder of the players in alphabetical order.

·          Older/lighter players must be clearly marked.

·          Jr. Midget and Midget teams and teams in associations with more than one team in a division i.e. two Jr. PeeWee teams in one association, must have at least 20 players on the roster with complete paperwork to be eligible.

·          All other teams must have at least 22 players on the roster with complete paperwork to be eligible.

·          Mitey-Mite teams are limited to a maximum of 33 players.

·          Teams with less than the required number of players with completed paperwork will not be eligible.

·          Background Check Affidavit for those volunteers identified after July 29, 2005.

·          Association Registration for Pop Warner National.

 

 

Sat August 19

PLAYER ELIGIBILITY:

 

Documents are to be in the following order.

 

1.        Pop Warner Little Panthers – Local Registration Form (ORIGINAL ONLY) – No other form is permitted.

·          Parental consent and medical release sections must be completed.

·          These forms are to be in the same order as the roster.

 

2.        Birth Certificate:

·          COPY of birth certificate.

·          Attach to the registration form.

 

3.        Scholastics:

·          Report card – Attach COPY of 2005 – 2006 full year report card to the registration form.

·          Scholastic Eligibility Form:  Attach Scholastic Eligibility Form to the Registration Form for players whose GPA is less than 2.0/70%, are “Home Schooled”, or whose report card is not available.  These forms must be completed and stamped by a representative of the school currently being attended by the player.

 

Thu August 24

 

Final league paperwork certification for Mitey-Mite, Jr. PeeWee, and PeeWee players; and second paperwork certification for Jr. Midget and Midget players whose paperwork is complete, and whose teams certified on August 19th.

 

Paperwork is to be turned in for those players whose paperwork was not previously submitted. Paperwork is to be turned in to the Executive Director.  The location and time will be announced.

 

Deadline:

 

Mitey-Mite, Jr. PeeWee, and PeeWee players with insufficient paperwork on this date will not be eligible.

 

 

Thu September 7

·          Laminated Identification Card:

 

·          Each player must have an Identification Card with all information completed.

·          A photo of the player must be permanently attached (no paper clips).

·          Identification Cards must be in the same order as the roster.

·          Any association whose Identification Cards are not ready for review by 6:30 pm on September 7th will not be eligible for post-season play.

 

 

 

COACH/VOLUNTEER ELIGIBILITY:

 

1. Background checks – Required prior to involvement with players:

 

·          Each association must complete a background check that meets Pop Warner requirements and complete and sign the Pop Warner affidavit stating that each coach and volunteer has passed the background check.

 

        2.     Laminated Identification Card – Required Thursday, September 7, 2005.

 

·          Each coach must have an Identification Card with all information completed.

·          A photo of the coach must be permanently attached (no paper clips).

·          Identification Cards must be in the same order as the roster.

 

 

Tue September 12

Final league paperwork certification for Jr. Midget and Midget players.

 

Paperwork is to be turned in for those players whose paperwork was not previously submitted.

 

Paperwork is to be turned in to the Executive Director.  The location and time will be announced.

 

Jr. Midget and Midget players with insufficient paperwork on this date will not be eligible.

 

 

Mon September 18

 

·          “Official Football Roster” with all information is to be turned in via the Pop Warner national software roster program.

Fri October 20

·          “Passing Progress Reports” or first quarter report cards are due to the Executive Director for participants on teams that are eligible for Regional competition and who were certified with the Scholastic Eligibility Form.  This requirement must be met in order for the participants to be eligible for the remainder of the season.
